Community Wellness Hubs

Community Wellness Hubs are trusted gathering places that connect community members to essential programs, services and spaces that advance health equity, improve health outcomes and enhance their quality of life.

As a partnership with Sacramento County Public Health, Health Education Council and The Center at Sierra Health Foundation, Community Wellness Hubs aim to address health disparities exposed by the COVID-19 pandemic and support communities in becoming more resilient. Community Wellness Hubs community-centered systems that connect County agencies, schools, libraries, hospitals and other essential partners to empower underserved populations to access information and care.

How Community Wellness Hubs Engages and Activates Community

Health Education

Prevention and early intervention workshops for optimum health

Disease Management

Addressing chronic disease, such as diabetes, hypertension and obesity

Screening

Risk assessments designed to address individual health and social risk factors.

Physical Health

Exercise for all to improve and maintain health.

Nutrition

Healthy eating and nutrition education for long-term health.

Mental Wellness

Mental health referrals, awareness, and support to address mental illness disorders including depression, anxiety disorders, schizophrenia, eating disorders and addictive behaviors.

Oral / Vision

Healthy teeth and vision for good health.

Social Wellness

Increased social connections through Community Wellness Hub group activities and gatherings.

Steps to Health Equity

Step 1

Residents interact with trusted Community Wellness Hubs: culturally relevant, health-focused partners.

Step 2

Assessments, pathways referrals, education, engagement, direct services, and quality care tracking delivered.

Step 3

Residents walk paths to great health with barriers to equity removed. Health outcomes achieved.

Pathways Program

Operating at Community Wellness Hubs, the Pathways Program is designed to address key health and social issues affecting at-risk populations. Through a health risk assessment, Care Coordinators identify barriers or risks to needed health and social services, assign “pathways” to each identified risk factor, and follow the community member until the pathways are completed.

Community Wellness Hubs Partners

Community Wellness Hubs partners with community-based organizations that are experienced and trained cultural brokers that understand implicit bias, patient trust, care access barriers, unequal healthcare quality and patient experience, and racism in clinical practices. These community-based organizations form the physical space for Community Wellness Hubs activities and the Pathways Program, which addresses key health/social issues for at-risk populations.
